What is the Thyroid (T3, T4, TSH) Test?

The Thyroid Profile (T3, T4, TSH) — also called a thyroid function test or TFT — measures the hormones that control your metabolism, energy, weight, mood and menstrual cycles.

Thyroid disorders are very common in India, especially in women. Hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid) are both easy to detect with this simple blood test.

What it measures: Total T3 (triiodothyronine), Total T4 (thyroxine) and TSH (thyroid stimulating hormone, made by the pituitary gland).

Parameters included (3)

  • Total T3 (Triiodothyronine)
  • Total T4 (Thyroxine)
  • TSH (Thyroid Stimulating Hormone)

Why is this test done?

  • Diagnosing hypothyroidism and hyperthyroidism
  • Monitoring thyroid medicine dose
  • Evaluating irregular periods, infertility or difficulty conceiving
  • Screening during pregnancy planning and in women above 35

When should you get tested?

  • Unexplained weight gain or weight loss
  • Tiredness, feeling cold, dry skin or hair fall (underactive thyroid)
  • Anxiety, palpitations, sweating or tremors (overactive thyroid)
  • Irregular or heavy periods
  • Swelling in the neck (goitre)

How to prepare for the Thyroid (T3, T4, TSH) Test

  1. 1No fasting is required.
  2. 2Give the sample in the morning, before taking your thyroid tablet (unless your doctor says otherwise).
  3. 3If you take biotin (vitamin B7) supplements, ask your doctor about stopping them 2–3 days before, as biotin can affect results.

Thyroid (T3, T4, TSH) Test normal range

ParameterTypical reference range
Total T380–200 ng/dL
Total T45.0–12.0 µg/dL
TSH0.4–4.5 µIU/mL

Pregnancy has different, trimester-specific TSH targets. Ranges are typical adult values and vary between labs, age and sex. Always use the range printed on your report and discuss results with your doctor.

Understanding your results

  • High TSH with low T4 usually means hypothyroidism; low TSH with high T3/T4 usually means hyperthyroidism.
  • A mildly raised TSH with normal T3/T4 is called subclinical hypothyroidism — your doctor will decide whether it needs treatment.
